| Please call to make appointments, request medication refills, or to ask
questions during regular business hours, 9:00AM-5:00PM, Monday-Friday.

| Emergencies |
| If you are having a medical emergency we suggest that you either call 911 or
proceed quickly to Norton Suburban Hospital.

| Canceling Appointments |
| We request that you give at least 24 hours notice if you need to cancel or
reschedule your appointment. We reserve the right to charge a nominal fee for
missed appointments.

| Refills |
| It is the policy of this practice not to refill medications or
prescribe narcotics after office hours. In an effort to ensure patients’
safety, we established this policy because we do not have ready access to
medical records when we are not in the office. Please call to make these
arrangements during regular business hours 9:00AM-5:00PM, Monday-Friday.

| When we are not in the office, our phones are answered by our
answering service. This service is for questions or concerns of an urgent
nature. If you are having a medical emergency call 911. |
